Insurance and Payments

We accept most commercial insurances, such as: Aetna, BlueCross BlueShield, Cigna, Tricare, United Behavioral Health, etc.


We do not accept Tenncare (Americhoice, Amerigroup, Bluecare, etc.), or Medicare.


If we do not take your insurance or your insurance does not include our providers in your network, we will also see clients on a cash pay basis. The amount is typically based on the adjustment that an insurance provider would accept. For more details about this option, please call the office.


The forms of payment accepted include check, cash, and most debit/credit cards. Our credit machine will not accept American Express or Care Credit cards. We are typically able to run FSA cards.


To set up an appointment

To set up your first appointment, please call (931) 490-0999 and ask to speak with our office manager to make an initial appointment. Please have your insurance card available to provide identification numbers and phone numbers for your insurance carrier. You will also be asked to provide a brief description of what you'd be coming in for, your address and phone number, as well as any medications you are currently taking.

At the time you schedule your appointment we are unable to quote exactly what your copay or coinsurance amount due at the first visit will be. We will call to verify insurance benefits before your first appointment and let you know the day before your scheduled visit with a reminder call for your appointment time.


The following self-assessment and intake forms are to be filled out by all new clients before the intake appointment. If coming in as a couple, each partner fills out his/her own forms.


If you are unable to complete your intake forms ahead of time or forget to bring them, you will need to arrive at least 15-20 minutes early. Extra forms are available at the front desk. The time allotted for the appointment cannot be extended due to incomplete forms.
